<p>A 15-year-old boy who was fatally shot in West Garfield Park last month has been identified. </p><p>Officers responded to calls of a person shot around 1 a.m. July 22 in the 4600 block of West Jackson Boulevard and found Lasiah Alexander Palmer with a gunshot wound to his head, Chicago police and the Cook County medical examiner's office said. </p><p>He was taken to Stroger Hospital where he was pronounced dead, police said.
